Reducing Falls in Resident with Dementia Diagnosis

Issue: A 70-year-old resident with a dementia diagnosis is lethargic and has recently experienced multiple falls. Lab work has shown to be within normal limits.

Solution: GuideStar Eldercare clinicians reviewed the resident’s medical history and diagnoses, and clarified current medical conditions. The medication review showed unnecessary medications had been prescribed without supporting diagnoses at the time, likely contributing to a loss of balance resulting in falls.

Results: GuideStar Eldercare wrote orders and gave recommendations to primary care that reduced the unnecessary medications and in turn resulted in a decrease in lethargy and falls by the resident.
